Cancellation Policy 

If you do not show up for your scheduled appointment and you did not notify us at least 24 hrs in advance, you will be required to pay $100. An Active credit card will be maintained on file for ease of payment. All fees (Patient's portion) are to be paid in full at the time of service. 

**Disability and FMLA forms will only be completed on established patients of 6 months unless extenuating circumstances.  

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check coverage carefully by asking the following questions: 

  1. Do I have mental health insurance benefits?

  2. What is my deductible and has it been met?

  3. How many sessions per year does my health insurance cover?

  4. What is the coverage amount for each visit?

  5. Is a referral required from my primary care physician? 

If your insurance is not accepted, a receipt called a SUPER BILL can be provided to you so that you can apply for reimbursement through your insurance company, if applicable.
